Does anyone have a form that they use for recording the distribution of digital photos?  I need to create a form for tracking photos of the work at a construction site so we know who's received what and as a cover page to go with the photos.  At this point I don't know if we will be sending them electronically or as a hardcopy.

Any help is appreciated. Thanks.

I might be able to get you a template of something our PM section use in paper format - would that be of any use?  If so let me know where to send it and I will do the bizz.

In a previous life I often had to send documents and technical drawings to construction offices on site.  These would be accompanied by a 'document transmittal' sheet - this was basically a simple form with a table that listed the drawing / document number, brief description, issue number (they were always being revised and superseded), and who created the document.  It may not relevant to you, but there was also a tick box for the purpose e.g. for information / for comment / please update your records, etc..  Finally, at the end of the page was a section that required the recipient to sign and return a copy as proof of receipt.  We then kept the signed copies, along with a copy of the original, in a central file that was divided up by site and contact (e.g. internal site office, client site office, sub-contactor's site office).  When the same documents were sent to multiple recipients the letter was addressed to the most important party (generally the client) with all other recipients cc'd at the bottom.

This was a few years ago so everything was on paper, but I'm sure it wouldn't be difficult to do a similar thing electronically.  For consistency we used the same form for everything sent to site be it drawings, documents, photos, equipment, etc..  Hope this gives you some ideas.
